Get Your 20th Anniversary Shirt!

In celebration of Whole Life's 20th Anniversary, Custom Tees will be sending out order forms for the latest Whole Life shirt design! Because emerald green is the color traditionally associated with 20th anniversaries, this shirt's design will have a hint of lustre green ink in addition to our usual blue and grey color scheme.

Sizes S - XL will cost $18.25, while sizes 2XL and up will be $20.25 a shirt. If you would like to place an order you can download and print the form below then return it to our administrative office at 1565 East State St. Hermitage, PA 16148. Full payment must be sent in with the order form.
